Adult Membership 2
13 and Older

- Professional dental cleanings (2 per year)
- Doctor exams (1-2 per year)
- Needed x-rays*
- Fluoride
- One emergency exam per year (if needed)
- 15% off all other treatments (some exclusions may apply)
- $150 Lifetime Activation Fee includes first month
- Total of $313 in annual savings
For patients with regular cleanings and absence of periodontal disease.
